Do I need to complete a health questionnaire for travel insurance?

A Medical Questionnaire (MQ) only needs to be completed for our Canadian Travel policies if you are 60 years of age or older. We can usually provide you with coverage even if you have some medical conditions. Be sure to complete the MQ as accurately as possible. If the questions are not answered truthfully and accurately, the traveller may be subject to an extra $15,000 USD deductible.

If you’re not sure about any medical conditions you may have, you can print a copy of the questionnaire and take it to your physician for assistance. You can find the Travel Insurance Medical Health Questionnaire attached below, or contact our office for a copy. 

We also offer an optional Unstable Pre-existing Condition coverage, if needed.
