If you’re unsure of the exact medical conditions you have, it is a good idea to contact your physician for assistance in completing the medical questionnaire (MQ).

It is extremely important to answer all of the questions accurately, as any errors or omissions could result in an additional $15,000 USD deductible being applied to any claim.  You can print out the health questionnaire, and complete it with your doctor. Then you can simply enter the answers in the Goose mobile app.
