- Liver or Kidney Conditions
- Hepatitis B or C
- Diabetes
- HIV or AIDS
- Liver or Kidney disease
- Circulatory or Blood Conditions
- Heart disease
- Stroke
- Transient Ischemic Attack or mini-stroke
- Disorder of the blood
- Neurological Diseases
- Paralysis
- Multiple Sclerosis
- Parkinson disease
- Alzheimer’s disease
- Huntington disease
- Amyotrophic Lateral Sclerosis (ALS)
- Total Loss of Speech, Vision, or Deafness
- Other neurological diseases not listed above.
- Cancer & related conditions
- For drug or alcohol use, has a medical professional ever recommended you to:
- Reduce consumption
- Undergo treatment
- Receive counselling
- Are you currently:
- Receiving disability benefits
- Unable to independently perform daily living activities (bathing, dressing, feeding, toileting)
- Confined to a hospital or health institution
- Recommended to undergo testing, investigations, or a specialist consult that is yet to be completed (including awaiting any test results)
- What is your height and weight?
* After first signing up as a Goose member and completing your profile, you become eligible to purchase these Group Life Insurance products on a Guaranteed Acceptance basis for the first 30 days after signup, meaning you will not need to fill out this health questionnaire to confirm eligibility during this initial period.
Future Guaranteed Acceptance periods may be offered on a promotional basis.
