How do I change my smoking status?

If you purchased Group Life, Critical Illness or AD&D insurance, you can update your smoking status from Smoker to Non-smoker as long as you have been a non-smoker for at least the last 12 months. 

In order for us to update your pricing & coverage, we will need you to sign a declaration. Please print the attached request form, complete and sign where indicated, and send the completed form to support@gooseinsurance.com. We will confirm your request and process the change.
